Reserve your spot with Island PT.
A limited number of new-patient consultations are available each month. Tell us about you and we will follow up to confirm your visit.
Here's how booking works.
Booking with us is not a fully automated system, and that is intentional.
Submit the form below with a few details about what brought you here. We will personally review your request, get back to you within one business day, and confirm a time that fits your schedule.
We want a quick conversation up front so we can make sure we are the right fit for what you are dealing with — and so your first visit feels prepared, not generic.

What happens after you submit.
Transparent about pricing.
No surprise bills, no insurance back-and-forth, no hidden fees.
We are a cash-pay practice, which means there are no surprise bills, no insurance back-and-forth, and no hidden fees. We will walk through the actual cost of your first visit and any treatment plan during our follow-up call — before you book anything.
Payment options we accept- All major credit and debit cards
- HSA (Health Savings Account) and FSA (Flexible Spending Account) funds

Want more detail on how cash-pay works and why we chose this model? Read our full breakdown on the Why Cash-Pay page.
Our cancellation policy.
Life happens — we get it.
If you need to cancel or reschedule, we just ask that you give us at least [24 / 48] hours' notice so we can offer the slot to another patient. Cancellations made with less notice may be subject to a fee.
If you are running late, give us a quick call or text. We will do our best to accommodate, though depending on the day your session may need to be shortened to stay on schedule for the next patient.
Frequently asked questions.
Six of the most common questions, answered honestly.
Do I need a doctor's referral to book?
No. Hawaiʻi is a direct-access state, so you can schedule physical therapy with us directly. (If you plan to submit a superbill for out-of-network insurance reimbursement, your plan may still require a referral — check with your insurance.)
How soon can I get in?
Most new patients are scheduled within a week of submitting their request. If you have something time-sensitive going on, let us know in the form and we will do our best to fit you in sooner.
Do I have to choose a service to book?
No. If you are not sure which service is the right fit, choose "Not sure — help me decide" on the form. We will figure it out together during our follow-up call.
Can someone else book on my behalf?
Yes — a family member, partner, or caregiver can submit the form for you. Just let us know in the "anything else" field who we should contact and how, and we will follow up accordingly.
What if I need to cancel after I book?
See our cancellation policy above. We ask for at least [24 / 48] hours' notice when possible. Reschedules are easy — just reach out.
Can I book a virtual consultation if I am not in Maui?
Yes. Virtual consultations are available for patients beyond Maui. They are especially useful for second opinions, follow-ups, and ongoing wellness check-ins.
You can reach us directly.
If filling out a form is not your style, no problem. You can reach us by phone, text, or email — whichever is easiest for you.

Take the first step.
Pain that has been around for weeks, months, or years rarely fixes itself. The fastest way to start moving past it is to actually take the first step — and that step is a short form. Submit your request, and we will take it from there.
