If you have only ever worked with physical therapists who bill insurance, the idea of a cash-pay practice can sound strange at first. Most people's first reaction is: "Won't that cost more?"

It is a fair question. But the honest answer is more interesting than yes or no — because once you understand how the insurance-based PT model actually works, the math starts to look very different.

Here is the short version: cash-pay means you pay us directly for your care, and we work for you instead of for an insurance company. That single change reshapes everything about how a session feels, how fast you get results, and how many visits you actually need.

02 / THE HIDDEN COST OF "FREE"

Why most people don't get better in insurance-based PT.

Insurance-based clinics have to run on volume.

A clinic gets paid per visit, and the reimbursement is lower than what private care actually costs to deliver. The only way the math works is to see more patients in less time.

That means:

  1. Sessions are usually 30 to 45 minutes — and the therapist might only spend 15 to 20 of those minutes actually working with you
  2. You may be double-booked, triple-booked, or moved between a therapist, an assistant, and a tech in a single visit
  3. Treatment follows protocols. Knee pain gets the knee protocol. Back pain gets the back protocol. Your individual body and history get less weight than the diagnostic code
  4. Visit counts get stretched out because each session only does a little. Twelve, sixteen, twenty visits is normal. Many people drop out before they finish
  5. The therapist's schedule, training, and treatment options are constrained by what insurance will reimburse — not by what would actually help you fastest

None of this is the therapist's fault. Most of them are excellent clinicians stuck inside a model that does not let them do their best work. We know because we have worked inside it.

03 / HOW OUR MODEL IS DIFFERENT

What changes when the insurance company steps out of the room.

When you remove the insurance company from the equation, three things change — and they change everything.

The therapist's time belongs to you.

Every session is the full appointment, one-to-one. Your therapist is not running between rooms, not finishing paperwork in their head, not watching the clock for the next double-booking. You get their full attention from start to finish.

Your plan is built around you.

No protocols, no insurance-approved treatment list. We treat what we find, the way we know works, based on your specific situation. That includes hands-on manual therapy, movement work, education, and the kind of detailed evaluation that takes time — because nobody is forcing us to rush.

You get better in fewer visits.

This is the part most people do not expect. Because each visit does more, you need fewer of them. A typical Island PT treatment plan is around eight focused sessions over six to eight weeks. Compare that to twelve, sixteen, or twenty visits at an insurance-based clinic — and the per-visit cost difference starts to disappear.

04 / DOING THE MATH

What this actually costs you.

Let's be straightforward about the numbers.

Insurance-based PT looks cheap on the sticker price — maybe a $30 to $50 copay per visit. But add up sixteen visits, factor in the deductible you have not yet hit, the time off work, the gas to drive to the clinic across town, and the fact that you may still not be feeling better at the end of it. Many of our patients have already been through that experience before they come to us.

Cash-pay PT has a higher per-visit price, but a clear total. You know what you are paying for. You know how long it will take. You know what you are getting at each visit. And when the plan finishes, it actually finishes.

We do not say this to bash insurance-based clinics. There are great therapists working inside that model. We say it because the model itself often gets in the way of the result, and our patients deserve to understand the trade-off they are actually making.

05 / HOW IT WORKS

What a cash-pay visit looks like.

i. Step 01

Reach Out.

Send us a message through the contact form or give us a call. We will get back to you, hear what you are dealing with, and answer any questions about the process before you commit to anything.

ii. Step 02

Your First Visit.

Your first appointment is a thorough evaluation. We take the time to understand your history, examine how you actually move, and identify what is driving your pain or limitation. By the end of that visit, you will have a clear picture of what is going on and a treatment plan to address it.

iii. Step 03

The Treatment Plan.

Most of our patients work with us for a focused treatment plan — typically around eight visits over six to eight weeks. Each session is one-to-one, and you pay at the time of service. We will tell you up front what the plan looks like and what it will cost so there are no surprises.

iv. Step 04 · Optional

The Wellness Membership.

Once your initial treatment plan is complete, you have the option to continue with our monthly wellness membership. This is for patients who want ongoing support to stay strong, mobile, and ahead of any future flare-ups. It is entirely optional — most patients who want it choose to add it after they have already experienced the difference.

06 / COMMON QUESTIONS ABOUT INSURANCE

Insurance, HSA, FSA, and reimbursement.

Most of our patients use HSA or FSA dollars. Many also submit for out-of-network reimbursement.

Can I still use my insurance?

We do not bill insurance directly, but we can provide you with a detailed receipt (called a "superbill") that you can submit to your insurance for potential out-of-network reimbursement. Coverage varies by plan, so we recommend calling your insurance company to ask about your out-of-network physical therapy benefits before your first visit.

Can I use my HSA or FSA?

Yes. Physical therapy is a qualified medical expense, so you can use HSA (Health Savings Account) and FSA (Flexible Spending Account) funds to pay for your sessions. This is one of the most common ways our patients pay.

What about Medicare?

Medicare has specific rules around cash-pay physical therapy. If you are a Medicare beneficiary, please reach out before scheduling so we can walk you through how this works for you specifically.

Do I need a doctor's referral?

In Hawaiʻi, physical therapy is direct-access. That means you do not need a doctor's referral to come see us — you can schedule directly. (Note: some insurance plans may still require a referral if you plan to submit for out-of-network reimbursement, so check with your insurance if that matters to you.)

Is cash-pay PT right for you?

We would rather be honest about it than waste your time. And ours.

Cash-pay physical therapy is a great fit for people who:
  • Have tried insurance-based PT and felt rushed, unheard, or stuck in the loop without real progress
  • Value their time and want to get out of pain in fewer visits, not more
  • Want one therapist who actually knows their case from start to finish
  • Have a high-deductible insurance plan where they would be paying out of pocket anyway until they meet their deductible
  • Want to use HSA or FSA dollars for high-quality care
  • Are willing to invest in their recovery the way they would invest in any other meaningful service

Cash-pay is probably not the right fit if you need to use only in-network insurance benefits with no out-of-pocket spending. If that is your situation, we are genuinely happy to point you toward in-network options on the island.
